DMV Pink Slips

DMV Pink Slip Request

To request for your Pink Slip, please fill out the Pink Slip Request Form here.

Students who are 15 1/2 (or older) and have successfully completed the Driver's Education/College Career class with a C or better may request a DMV Pink Slip. Pink slips can be picked up ONLY on Wednesday from 8-4 in the COUNSELING office. The information must be completed thoroughly; FULL name must match your birth certificate

**Students that took College/Careers in the Spring of 2020 are not eligible for a pink slip**
